2. Original Certificate SSLC (Original & copy)
3. Original BPT Mark lists (Original & copy)
4. Internship Completion Certificate (Original & copy)
5. Plus 2 Mark List (Original & copy)
6. Conduct Certificate (Original & copy)
7. Transfer Certificate from the institution where the candidate has studied last
8. Passport size photo 5
9. Stamp size Photo 2
10. Copy of Aadhar
11. Students must submit Migration Certificate (if the BPT Certificate is other than KUHS)
12. Medical Fitness Certificate from Registered Medical practitioner
13. Eligibility Certificate obtained from the Medical university (KUHS) for candidates who studied in other universities outside Kerala & abroad
14. Payment of the Fees (see the prospectus)
